Direct Service Professional

Job Description

 

  1. POSITION OBJECTIVE- HELP CONSUMERS ACHIEVE THEIR PERSON CENTERED GOALS.

The Direct Service Professional (DSP) is a first line position for JST Community Services and is responsible for assisting, supporting and teaching the skills necessary to enable consumers to achieve their daily living, community participation, leisure and social skills goals.

 

  1. MAJOR RESPONSIBILITIES

 

  1. Person Centered Support Planning (PCSP) Implementation and Training
  2. Assist consumers in working towards their goals and taking all necessary documentation related to goals.
  3. Provide input into the development of services, goals and objectives.

 

  1. Treatment/Monitoring of Medical Concerns
  2. Provide direct training to consumers in order to maintain health/wellness and acceptable hygiene practices.
  3. Monitor and document health information and concerns on a daily basis.
  4. Notify Manager of any medical issues or concerns.
  5. Assist consumers to administer their medications and provide training to increase their skills in self-medication.

 

  1. General Duties
  2. Assure site upkeep, maintenance, safety and sanitation in accordance with JST policy.
  3. Assist in transportation to and from day service providers and activities in the community as required.
  4. Assist consumers with connecting to recreation/leisure activities in the community.
  5. Follow all JST Policies and Procedures.
  6. Assure that all company, employee and consumer information is held confidential according to JST policy.
  7. Assure that consumers are free from abuse, neglect and exploitation.
  8. Other duties and responsibilities may be assigned.
