Implements the Plan of Care provisions to meet their needs, which include: social service goals for alleviating problems, supportive counseling, problem solving, community referrals pre-bereavement and bereavement care. Regularly re-evaluates the patientapos;s and familyapos;s psychosocial needs. Adheres to Company policies and procedures and regulatory compliance requirements and directives. Requirements: Requirements * Minimum age requirement of 18.* Bachelorapos;s degree in Social Work, Masterapos;s degree preferred. Masters degree from school accredited by the Council on Social Work Education required in states as specified.* Licensure and/or certification as required by the State.* Minimum of two yearapos;s social service experience in a health care setting or social services agency.* Knowledge of and ability to obtain community resources (information and referral, state/federal financial programs and eligibility requirements, etc.).* Compliance with accepted professional standards and practices.* Understands Hospice philosophy, principles of death/dying.* Ability to work in an interdisciplinary setting.* Physical exam in accordance with state law requirements.* Valid driverapos;s license and an automobile that is insured in accordance with state and/or Odyssey requirements and is in good working order.* Satisfactory references from employers and/or professional peers.* Satisfactory criminal background check.* Compliance with accepted professional standards and practices.* Ability to work within an interdisciplinary setting.* Demonstrate excellent observation, judgment and communication skills.
